One CHC tracks implementation, changes, and responsibilities associated with policies and procedures in a spreadsheet. This spreadsheet is managed by a clinical medical assistant, a representative from HR, and a representative from the finance department. Each policy/procedure has a review date. All policies must be updated every 3 years, at the longest, but most are reviewed annually if not more frequently.
