Designing a PCMH development plan. There are seven suggested steps:
- Convene PCMH development TEAM
- Schedule regular PCMH development TEAM meetings
- Determine goals for PCMH development
- Complete a Readiness Assessment and identify gaps in PCMH core capabilities
- Action Planning – Identify a Pathway to PCMH Implementation
- Develop a workplan and timeline to accomplish your goals, including responsibilities for TEAM members
- Engage your entire clinic team, Re-assess status regularly, Celebrate successes!
