The NCQA PCMH Self Assessment Tool is an organizational management tool to be used by PCMH leaders to keep track of the criteria and documentation that has been put into place. We recommend that health centers use this tool to mark NCQA PCMH criteria and documents as they are implemented and developed. The summary on the “SCORE” worksheet will help you track overall “progress” in terms of number of core and elective criteria put into place thus far. (Please Note: This tool replaces the CHS PCMH Tracker)
